What Happens Next?
After filling out the form on this page, a patient care specialist will provide a free pre-screening to assess your eligibility for medical marijuana treatment in your selected state. If you meet the initial criteria, we’ll schedule an appointment with one of our qualified physicians. If the physician confirms your eligibility, they will register you in the Medical Marijuana Use Registry, allowing the Department of Health to issue your certification.
